Forms for Camp

Below are links for forms and information files for Camp.  Some forms are required by the state of Michigan.  Please review the descriptions of each to see if they pertain to your camper.

These files are in the PDF format and can be viewed on line or printed to be mailed to Michindoh.

Health Form
We must have a "Health Form" on file for all campers.  If you did not register on line or you did not fill out the online "Health Form", please use this form.

Camper Profile
The "Camper Profile" form is not required, but it will help our cabin counselors get to know your child.

Horse Riding Waiver
This waiver is required for all campers who will be riding horses while at camp.  (Note only some sessions offer riding)

Information on how to send email messages to your child and how to purchase photos from your child's week at camp.
